The Ownership
Investigation Toolkit

Federal data on every owner, operator, and chain — with the citations, exports, and network views you need to build a story.

All data is sourced from CMS public datasets — the Provider Data Catalog, CASPER ownership disclosures, Special Focus Facility program, and Civil Monetary Penalties system. Every claim on this site links back to a dated CMS record. Verify at CMS.gov

Four angles worth investigating

Not rankings. Not verdicts. These are filtered views into the data — starting points for your own investigation.

Multi-state actors

Owners cited across multiple states

Same individuals or corporate shells appearing in CMS O-0 disclosures across state lines — often the footprint of a roll-up operation or a management company spanning regional markets.

Open query
Active SFF designations

Operators tied to active Special Focus Facility designations

Facilities currently under CMS's most intensive oversight. When the same operator appears on this list more than once, that's the story.

Open query
Active roll-ups

Chains that have acquired facilities in the last 18 months

Recent acquisition activity visible in CMS ownership change disclosures. These are the operators growing their footprint fastest — the ones worth tracking before the next enforcement cycle.

Open query
REIT ownership

REIT-owned facilities, by state

Real estate investment trusts own a significant share of U.S. nursing home real estate without appearing in the operator record. The macro view shows where REIT ownership concentrates geographically.

Ownership Network

The relational view. See how individuals, LLCs, and chains connect across thousands of facilities. Built from CMS CASPER disclosure data.

How to cite Placet

Placet, "Nursing Home Ownership & Enforcement Data," sourced from CMS Provider Data Catalog [accessed Month DD, YYYY], placet.com.

Every data point links back to a dated CMS source. We recommend citing both Placet and the underlying CMS record.

Methodology

All ownership data is parsed from CMS CASPER Form CMS-855A and O-0 ownership disclosures. Enforcement data from the CMS Civil Monetary Penalties dataset. SFF designations from the active CMS Special Focus Facility list.

Full methodology

A note on entity resolution

Names in CMS O-0 ownership disclosures are not yet entity-resolved with the confidence required to publish per-person rankings. "John Smith" at one LLC is not necessarily the same "John Smith" at another. We are actively working on this. In the meantime, every claim on every page links back to a dated CMS source — use those records as the authoritative reference in your reporting.

The network graph is most reliable at the organizational level — corporate chains, management companies, and REITs — where name matching across CMS records is more robust.
