All ownership data is parsed from CMS CASPER Form CMS-855A and O-0 ownership disclosures. Enforcement data from the CMS Civil Monetary Penalties dataset. SFF designations from the active CMS Special Focus Facility list.
Full methodologyA note on entity resolution
Names in CMS O-0 ownership disclosures are not yet entity-resolved with the confidence required to publish per-person rankings. "John Smith" at one LLC is not necessarily the same "John Smith" at another. We are actively working on this. In the meantime, every claim on every page links back to a dated CMS source — use those records as the authoritative reference in your reporting.
The network graph is most reliable at the organizational level — corporate chains, management companies, and REITs — where name matching across CMS records is more robust.
